Open dark souls 3 exe in this program, the exe will be found in your steam game folder location - Program Files(x86)\Steam\SteamApps\common\DARK SOULS III\Game

Use a hex editor like this – https://mh-nexus.de/en/hxd/

Then 'CTRL F' and search for 80 07 00 00 38 04 and once found you then need to replace that with either of the following numbers

For 2560x1080 you want 00 0A 00 00 38 04
For 3440x1440 you want 70 0D 00 00 A0 05

Then save and quit program

Now navigate to AppData\Roaming\DarkSoulsIII (you can just type %appdata% into windows explorer and then go to dark souls 3 folder that way).
Then load up the file GraphicsConfig.xml into notepad/notepad++ and edit the following

config ScreenMode WINDOW /ScreenMode
Resolution-WindowScreenWidth 2560 /Resolution-WindowScreenWidth
Resolution-WindowScreenHeight 1440 /Resolution-WindowScreenHeight

And change it to

config ScreenMode WINDOW /ScreenMode
Resolution-WindowScreenWidth 3440 /Resolution-WindowScreenWidth
Resolution-WindowScreenHeight 1440 /Resolution-WindowScreenHeight

Now save file and exit. Then right click it and go properties, and make tick read only. Now close pop up window, this stops the game changing it later.

Now load up the game, and make sure its fullscreen and correct resolution and boom you are off.
It always loads in a window so you just need to each time make it fullscreen in the settings and you are good to start playing
